survey engine: support textarea and a ranked list

It would be useful for the survey engine to support a larger text input for open-ended questions. I added a bit to the CSS to give the text inputs 100% width, but it's still not large enough.

I've also had a need for the ability to order/rank the possible answers.

If you want a more advanced survey you should try the demo of my add on product Form Wizard Pro.

I have no plans for improvements to the free survey feature included in mojoPortal because that would be competing against my product. Sales of my add on products are what make it possible for me to work on mojoPortal full time.
