Hi Thomas,
Its going to be at least a few weeks before I can make any more video tutorials, but basically the steps are:
1. Create a survey
2. Add 1 or more pages to the survey
3. add 1 or more questions to each page
After you create a survey you see a link for adding pages
Ater you create a page you see a link for adding questions
You can choose form several question types. If you choose Radio Button list or CheckBox list you wil be able to add options.
