In the survey field edit page there is an error in line 132 which causes options to be visible for Date fields. The line states:
if ((QuestionType)question.QuestionTypeId == QuestionType.TextBox || questionType == QuestionType.Date)
and should state:
if ((QuestionType)question.QuestionTypeId == QuestionType.TextBox || (QuestionType)question.QuestionTypeId == QuestionType.Date)
